First visit: Please bring a valid ID, insurance information, and any relevant medical records.

Re-visit: Update any changes in your personal information or insurance coverage at the front desk.

Insurance: We accept most insurance plans. Please bring your insurance card, and co-payments are due at the time of service.

Self-Pay: If you are self-paying, full payment is due at the time of service. We accept cash, credit cards, QR code and IC cards.
